Night guard dental code

D9944, D9945 and D9946, what each covers, and what a guard costs with or without a plan

A night guard is billed as an occlusal guard. The code is D9944 for a hard guard covering the full arch, D9945 for a soft guard, and D9946 for a hard guard covering part of the arch.

The old single code D9940 was retired in 2019, and plans that still list it usually map it to D9944. The code tells your plan what the appliance is. The price is a separate number, $119 an arch for a custom night guard made from the mold you take at home.

Does insurance cover a night guard

Many plans cover an occlusal guard once every three to five years, at 50 to 80 percent after the deductible, and only when a dentist records grinding or clenching. Some plans exclude guards entirely. The only way to know is to call the number on your card with the code.

If the plan pays nothing, a $119 guard is usually less than the copay would have been.

HSA and FSA

A night guard is HSA and FSA eligible as a dental appliance, with or without a code. Pay with the card at checkout, keep the itemized receipt, and check with your plan administrator if you want it in writing.

Which guard matches which code

  • Hard guard, D9944. Designed for heavy grinders, lasts longest, the most common custom order.
  • Soft guard, D9945. Designed for light clenchers who want cushion, wears faster.
  • Hybrid, hard outside and soft inside. Billed as D9944 by most offices.
